PrimeStar Boost Dental Plan

Ameritas PrimeStar Boost Dental Plan is their middle option. It gives you more coverage during the first year if you need more than just Preventative Care.

PrimeStar Complete Dental Plan

 Ameritas PrimeStar Complete Dental Plan offers the most coverage on certain Basic and Major services during the first year of benefits.

FCL Copayment Q & QF PPO Dental Plans

  • This product offers Pediatric Dental Benefits only with no wait periods and a copayment schedule of benefits.

  • A Dental Plan for adults only or the whole family with a copayment schedule.

FCL Choice Q & QF PPO Dental Plans

  • This product offers Pediatric Dental Benefits only with no wait periods and a coinsurance schedule of benefits.

  • A Dental Plan for adults only or the whole family with a coinsurance schedule. The FCL Choice network is the larger network of FCL’s two PPO plans.

FCL BlueCare HMO Dental Plan

  • FCL’s BlueCare HMO Dental Plan has no Wait Period and no Cap on services.
